ESSA Membership Terms

These Membership Terms outline the terms and conditions of participation in the ESSA Beverly Hills membership program.

Membership Selection

The member is enrolling in one of the following membership levels, as selected at the time of enrollment:

ESSA Refresh
ESSA Refine

Membership benefits, eligible treatments, preferred pricing, and monthly wallet credits vary based on the membership level selected and are described in the applicable membership offering at the time of enrollment.

Membership Structure

Memberships are billed monthly on the member’s enrollment anniversary date.

There is no enrollment fee, minimum commitment, or long-term contract. Memberships continue on a month-to-month basis until cancelled.

The applicable monthly membership fee will be automatically charged to the payment method on file.

100% of the monthly membership fee is deposited into the member’s ESSA wallet as treatment credit in accordance with the selected membership level.

ESSA Refresh

ESSA Refresh is designed for ongoing wrinkle relaxer maintenance.

Benefits may include:

·Monthly wallet credit

·Exclusive member pricing on eligible wrinkle relaxer treatments

·Priority booking when available

·Access to select member-only promotions and offers

ESSA Refine

ESSA Refine is designed for patients who want broader access to ongoing aesthetic maintenance.

Benefits may include:

· Monthly wallet credit

· Exclusive member pricing on eligible wrinkle relaxer treatments

· Exclusive member pricing on select additional aesthetic treatments

· Priority booking when available

· Access to select member-only promotions and offers

Specific eligible services and member pricing may change from time to time and will be reflected in the current membership offering.

Membership Credits

Monthly membership credits are deposited into the member’s ESSA wallet.

Unused wallet credits roll over and remain available for future eligible treatments.

Membership credits:

· May only be used toward eligible services provided by ESSA Beverly Hills

· Are non-transferable

· Are non-refundable

· Have no cash value

· Cannot be redeemed for cash

Unused purchased wallet credits remain available after cancellation of the membership, subject to the terms of this Agreement.

Member Pricing and Benefits

Member pricing and benefits are available only while the membership is active and in good standing.

Member pricing applies only to eligible treatments and may not be combined with other promotions, discounts, packages, or special offers unless specifically stated.

Treatment recommendations and eligibility remain subject to medical evaluation and provider discretion.

Membership does not guarantee that any particular treatment is medically appropriate for the member.

Automatic Payment Authorization

By enrolling, the member authorizes VS Medical Aesthetics Nursing PC to automatically charge the payment method on file for the applicable monthly membership fee on each recurring billing date until the membership is cancelled.

The member is responsible for maintaining a valid payment method.

If a payment is declined or cannot be processed, membership benefits may be suspended until the account is brought current.

Cancellation

Memberships may be cancelled at any time.

Cancellation must be completed at least 7 days before the next scheduled billing date to prevent the next recurring charge.

Cancellation may be requested through the available Client Portal cancellation feature or by contacting ESSA Beverly Hills.

Membership fees already processed are non-refundable.

Upon cancellation:

· Future recurring membership charges will stop

· Membership pricing and active-member benefits will end at the conclusion of the current paid billing period

· Any unused treatment credit already purchased and deposited into the member’s ESSA wallet will remain available for eligible treatments after cancellation.

There is no early termination fee.

Treatment and Medical Care

Membership enrollment does not guarantee treatment.

All treatments remain subject to an individual medical assessment, applicable contraindications, clinical judgment, and the member’s informed consent.

The provider may recommend against, postpone, modify, or decline a treatment when medically appropriate.

Results from aesthetic treatments vary between individuals, and no specific outcome or duration of results is guaranteed.

Changes to Membership

ESSA Beverly Hills reserves the right to modify membership pricing, benefits, eligible services, or program terms with reasonable notice and in accordance with applicable law.

Changes will not convert existing wallet credits into cash or eliminate credits already purchased and deposited into the member’s account.

The membership program may be discontinued or a membership may be terminated for nonpayment, misuse of membership benefits, inappropriate conduct, or violation of these terms.

Acknowledgment

By enrolling in an ESSA membership, the member acknowledges that they:

· Have reviewed and understand this Membership Agreement

· Understand the benefits associated with the membership level selected

· Authorize recurring monthly billing

· Understand the cancellation policy

· Understand that membership credits are non-refundable and have no cash value

· Understand that all medical treatments remain subject to provider evaluation and clinical judgment

By completing checkout, the member authorizes the initial membership charge and recurring monthly charges to the payment method on file until the membership is cancelled in accordance with this Agreement.
